Shalom Play Registration

By this waiver and release, I agree to the following:

I represent that I am the parent or legal guardian of the child(ren) named above or have obtained permission from the parent/legal guardian of the child(ren) named above to execute this Agreement on their behalf.

I agree that I will follow the rules and procedures of the program and the verbal instructions of staff, and ensure the child(ren) named above also follow those rules, procedures, and verbal instructions.

I agree that by bringing the child(ren) named above to the program, I confirm that they are healthy, illness-free, and are able to safely participate in the program activities.

I agree that I assume all risks and hazards associated with participation in this program and agree to indemnify Temple Beth Abraham, Shalom Play, PJ Library, their staff, officers, volunteers, and affiliates, for any defense cost or expense arising from any and all claims, injuries, liabilities, or damages arising from participation, except for those arising from the gross negligence or willful misconduct of Temple Beth Abraham.

I understand that refunds will not be issued for classes missed, and that requests for up to two makeup sessions may be granted on a case-by-case basis, at the sole discretion of Temple Beth Abraham and Kindergym staff, as arranged by by Kindergym staff when capacity allows.

I understand that Temple Beth Abraham staff reserve the right to require participants to leave a session and/or the program at any time, at their sole discretion.

I understand that photos may be taken during Temple Beth Abraham and PJ Library programs which may be posted for promotional use without names or identifying information.
